Below are the links to the forms you will need to apply as a new agency. In order to consider your agency for distribution status with the Midwest Food Bank, we request you submit the Agency Application and the Terms of Donation forms. In addition, we'll need a copy of your 501c3 letter showing non-profit status. If you are a church, in lieu of the 501c3, we will accept a letter (on church letterhead) signed by the lead pastor that verifies the name of the agency and the non-profit status of the church.
